What is a Family Health Team? (FHT)

A Family Health Team is an approach to primary health care that brings together different health care providers to co-ordinate the highest possible quality of care for you - the patient.

The Family Health Care Team is designed to give doctors support from other complementary professionals.  Most Family Health Teams will consist of doctors, nurses, nurse practitioners and other health care professionals who work collaboratively, each utilizing their experience and skills, so that you receive the very best care, when you need it, as close to home as possible.  For more information about Family Health Teams, please visit:

Are there any fees associated with the participation in programs and services offered?

All programs and services are available at no charge to rostered patients of the Prime Care Family Health Team.

Are there After Hours Clinics?

We offer weekday evenings and Saturday Clinic hours.  See the After Hours Care section for Clinic hours.

Where is the closest lab to complete blood work?

The closest lab is Life Labs which is conveniently located in our building.

What is the Telephone Health Advisory Service (THAS)

The Telephone Health Advisory Service (THAS) is an after-hours telephone health advice and triage service for enrolled/registered patients of a Family Health Team (FHT) in Ontario.  For more information, see the Telephone Health Advisory Service section.


Do I require my Health Card with each visit?

You will need to present your Health Card at the Reception Counter with each visit.  This will ensure that your Health Card information is up-to-date in our system.  Please advise us if there has been a change in address and/or telephone number as well.

If I am seen by an Allied Team Member of the Family Health Team, will my family physician be aware of my visit and receive any relevant results?

Yes.  Our Family Health Team uses an EMR system (Electronic Medical Records), and as a result, all information in our system can be viewed by your physician.
