CHRONIC DISEASE MANAGEMENT (CDM) PROGRAMS
Through this group education program, patients who have arthritis are invited to learn more about the preventive measures such as consuming a nutritious, well-balanced diet, maintaining a healthy weight and including physical activity into their daily routine to help manage and reduce the progression of arthritis.
Class is lead by the Dietitian and a Physiotherapist from The Arthritis Society. One-on-one follow up appointments are available as needed.
This group education program invites patients with Type 2 Diabetes to participate in a group setting where they will learn all they need to learn about their diagnosis. The goal of this program is to provide patients with the education to facilitate self-management so the patient can manage their Blood Glucose daily. The common topics covered include pathophysiology, symptom management, medications, healthy meals, physical activity, weight management, depression and insulin therapy.
Class is lead by the Nurse Educator, Dietitian, Mental Health Counselor and Pharmacist. One -on-one follow up appointments are available as needed.

This group education program focuses on educating patients with pre-diabetes (Impaired Glucose Tolerance and/or Impaired Fasting Glucose), or those who have a family history of diabetes. The program will incorporate group sessions where you will learn more about pre-diabetes and how lifestyle changes can help reduce the progression to Type 2 diabetes. Participation in this program will help you to reduce your blood glucose levels by increasing your physical activity, enjoying healthy meals and losing weight.
Class is lead by the Dietitian, Nurse Educator, Mental Health Counselor and Pharmacist. One-on-one follow up appointments are available as needed.

This group education program is targeted to aid patients who have heart disease (high blood pressure, elevated cholesterol or previous heart problems) or who have a family history of heart disease. The goal of this program is to educate the patient how diet, lifestyle and medication management can help reduce the risk of developing, or the progression of, heart disease. This program will provide information about how to change modifiable risk factors like stress, diet, smoking, obesity, and physical activity. Specific aspects of a heart healthy diet and how to incorporate tolerable physical activity will be discussed. Learning more about your heart medications and potential interactions will also be reviewed.
Class is lead by the Dietitian and Nurse Educator. One-on-one follow up appointments are available as needed.

Pregnant? Looking for some information to prepare you for pregnancy and childbirth? Our prenatal classes offer you and your support person with a welcoming environment to prepare you for labour, birth and early parenthood. Join others like you in learning how your baby is growing, different birthing methods and positions, the role of your support person, relaxation techniques, pain relief choices, details of medical procedures and interventions, and of course breathing techniques. Other information can include information related to circumcision, breast and bottle feeding, and basic newborn infant care, including how to change a diaper!
Classes will be led by the Nurse Educator and Nurse Practitioners.

COPD is short for Chronic Obstructive Pulmonary Disease. COPD symptoms often slowly worsen over time. However, sometimes they can worsen suddenly. For instance, a cold, the flu, or a lung infection may cause a ‘lung attack’ making it much harder to catch your breath. When these attacks happen, you may have chest tightness, more coughing and changes in the color or amount of your sputum (spit). The respiratory therapist will provide instructions on how to treat a ‘lung attack’, what breathing exercises to do and what inhalers to take – all to help you breathe easier.
The goals of the COPD Program include: Relieving your shortness of breath, slowing the progression of the disease, improving your exercise tolerance, preventing and treating complications and improving your overall health.
Exercise is proven to keep people with COPD healthy; therefore, all patients in the COPD Program will be referred to a physiotherapist who will provide you with several exercise options, including a local medically supervised Pulmonary Rehab Program.
Quitting smoking is the most important step you can take to treat COPD. The multidisciplinary team can help you quit.

Asthma is a chronic lung disease that inflames and narrows the airway. It is fairly common and can be diagnosed at any age. The common asthma symptoms are: cough, wheeze, chest tightness and shortness of breath. These symptoms tend to come and go. If the symptoms occur at night, first thing in the morning, or during exercise, then the asthma is not well controlled. Asthma is well controlled when you are:
· Exercising without symptoms
· Sleeping without symptoms
· Not needing to use your reliever inhaler (Blue puffer) more than three times a week
· Not missing school or work

Spirometry is a simple breathing test that is now available at Prime Care Family Health Centre.
Who should have spirometry? Take the Canadian Lung Health Test to find out.
· Do you cough regularly?
· Do you cough up phlegm regularly?
· Do even simple chores make you short of breath?
· Do you wheeze when you exert yourself? Or at night?
· Do you get frequent colds that persist longer than those of other people?
If you answered yes to any one of the above questions, talk to your doctor or Nurse Practitioner about taking a simple breathing test called spirometry.

· Patients are referred to the program by their family physicians, the physician’s assistant, the nurse practitioners, or the registered nurses within the Prime Care Family Health Team.

Components of the program:
With the program Break Through, an Education Program for the Prevention, Treatment and Management of Osteoporosis, the various health care providers can provide the following services;
· Basic information regarding your bones
· Help you assess your bone health
· Information regarding proper nutrition and bone health
· Information regarding exercise and movement for bone health
· Treatments for osteoporosis
· Strategies to help you live with Osteoporosis
· Strategies to make you safe from falls

· This program is intended for the patients of the Prime Care Family Health Team with the diagnosis of Heart Failure.
· Patients are referred to the program by their family physicians, the physician’s assistant, the nurse practitioners, or the registered nurses within the Prime Care Family Health Team.
Program location:
· Prime Care Family Health Team location at 110-470 Bronte Street South in Milton.
Components of the program:
The various health care providers can provide the following services;
· Physical examination and different types of testing when needed to monitor the patient’s condition.
· Information and education on various topics in order to better manage the patient’s condition.
o Diet
o Exercise
o Weight management
o Salt and fluid restriction
o Quitting smoking
o Annual flu and pneumonia vaccinations
· Treat other health conditions such as; Diabetes, High blood pressure and High cholesterol which may affect their condition.
· Provide information regarding medications used to treat Heart Failure as well as have the opportunity to discuss his/her questions or concerns with a pharmacist.
· Counseling services to discuss issues related to his/her health condition.
HEALTH PROMOTION & DISEASE PREVENTION PROGRAMS
Knowing more about your usual food intake and exercise habits will help you, your dietitian or nurse educator determine what changes, if any, may improve your health condition and promote overall wellness. When we record what we are doing, we become accountable to ourselves as well as to our health care provider. Feel more in control of what you are doing to help achieve your weight loss goals or manage your diabetes, blood pressure and cholesterol levels. With your dietitian or nurse educator, learn how the foods you eat and the exercise you do affects your blood sugar, cholesterol or blood pressure levels.
To prepare for your upcoming appointment with the dietitian or nurse educator, we ask that you fill out a minimum of 3 days of your usual food intake and physical activity routine. You will have a 2 week free trial of the Nutribasic service to complete your food and exercise information. During this time, you are welcome to use the other features of the service including pre-established meal plans, medication tracker, and logs for your blood pressure and blood sugar readings. This information is electronically secure and will not be shared by anyone besides your health care provider.
At your appointment, you will receive a copy of the results of your data which will be used to create personalized lifestyle modifications JUST FOR YOU!

Through this group education program, patients who want to lose weight (to prevent health issues or to help manage current conditions) will increase their knowledge and enhance their skills with their diet and physical activity goals to help make positive changes for optimal health and wellness. Topics include: achieving a healthy weight, proper eating habits, getting adequate nutrients to meet daily needs (including sodium, fat, fibre, and carbohydrates), how to incorporate physical activity into your routine and how lifestyle changes can prevent various chronic conditions like diabetes, heart disease, arthritis and cancer as well as helping to manage existing conditions and improve quality of life.
Class is lead by the Dietitian and Nurse Educator. One-on-one follow up appointments are available as needed.

Parents are assisted with both feeding and nutritional concerns from newborns to five years of age. Issues with breast or bottle feeding, transition to solids, transition to table foods, picky eaters, vegetarian babies, and children with diagnosed food allergies can be seen. Concerns about slow growth, anemia (low iron) and constipation are also addressed. A referral to see the pediatric nurse practitioner can be made through you physician's office.

Kick Butt!
It’s the smoking cessation program that really works! We offer a comprehensive program that combines addiction treatment with the support you need to lead a smoke-free lifestyle.
Our program includes:
1. Assessment and developing a plan.
2. 4-5 days post quit date: behavioural counselling and education.
3. 3 weeks post quit date: counselling and relapse prevention planning.
4. Follow-ups as needed by you.

This program is for patients 55 years and older. Patients are seen on a referral basis for cognitive testing by a Nurse Practitioner and/or Registered Nurse. Patients who are interested in this program may contact their family physician’s office for a referral. We educate seniors about resources and services available by the Interdisciplinary Health Team Member within the Family Health Team. We provide assistance to seniors to access community safety assessments and resources. This program provides healthy eating resources including literature and group sessions. We offer assistance and support to caregivers of our senior patients to access community resources. The Registered Nurse provides a Home Visiting Program for seniors who are not able to visit their family physician regularly.
