Nurse Practitioners:

A Nurse Practitioner (NP) is a registered nurse with advanced university education who provides personalized health care with a focus on assisting clients to be as healthy as possible with emphasis on disease prevention and healthy living. Services include general assessments, pre & post natal care, pre-conception counseling, infant/child nutrition and development issues, breastfeeding support, birth control counseling, immunizations, breast exams and PAP tests. NP’s can also diagnose and treat minor illnesses and injuries. In collaboration with your physician, the NP can manage chronic illnesses and provide prescription renewals.
Physician Assistant:
Marc Tobin
A Physician Assistant (PA) is a highly skilled allied health professional who practices in a physician extender role. They perform tasks similar to those preformed by their physician supervisors which would include taking a patient history, performing physical examinations, ordering diagnostic testing, referring patients for specialist care, diagnosing medical conditions and treatment including prescribing medications within the scope of their practice.
Registered Nurse:
Julie Cordasco
The Registered Nurse holds a degree in nursing. She provides general health screening services such as blood pressure checks and immunizations. She educates, empowers and advocates for patients. Our Registered Nurse is involved with the Seniors Wellness and Education for Living Life (SWELL) Program which includes a Seniors Home Visiting component.
Mental Health Counsellors:

The Mental Health Counsellors provide assessment and counselling for psychosocial and mental health problems. Services include strategies to cope with depression and/or anxiety; to manage stress more effectively; grief counselling; learning how to build self confidence; and more. Additional services provided include the Smoking Cessation Program.
Psychologist:
Dr. Lynn Crawford
The Clinical Psychologist is trained in assessment, treatment, and prevention of behavioural and mental conditions. Services include diagnosis and recommendations for treatment, as well as providing cognitive behavioural therapy (CBT). CBT is a psychological treatment that addresses the interactions between how we think, feel and behave. It is usually time-limited, focuses on current problems, and reinforces positive behaviours.
Registered Dietitian:
Pamela Piotrowski
The Registered Dietitian is uniquely trained to offer individual medical nutrition counselling to help manage diabetes, hypertension, high cholesterol and many other health issues. The dietitian offers science-based diet and nutrition information as well as practical solutions for healthy eating, weight loss and preventing various chronic conditions. Group sessions are also offered to increase knowledge and skills on a variety of topics including: good and bad fats, where to find fibre, menu planning, label reading and grocery shopping.
Nurse Educator:
Safa Masri
The Nurse Educator provides expertise in the prevention, screening and management of chronic diseases such as diabetes, hypertension and hyperlipidemia. She focuses on providing health education on a broad range of topics including those related to health promotion and disease prevention to both individuals and groups. This education is given with one main goal in mind: to put the patient back in control of their health.
Respiratory Therapist:

There is one Registered Respiratory Therapist (RRT) on staff who is licensed by the College of Respiratory Therapists of Ontario (CRTO) and is a Certified Respiratory Educator (CRE). Respiratory Therapists are health care professionals who monitor, evaluate and treat individuals with respiratory and cardio-respiratory disorders. The respiratory therapist works with the multidisciplinary team to diagnose, educate, and provide self-management tools to patients with respiratory disease such as Asthma and COPD (Chronic Obstructive Pulmonary
Disease).
Chiropodist:
Roshni Patel
Our Chiropodist is here to provide comprehensive assessment and treatment plan for patients with diabetic foot conditions in a primary care setting. There are not many foot clinics available, leaving many diabetics with little options. Our new Diabetic Foot Care Clinic addresses issues related to diabetes which gives FHT patients an option other than the emergency department and reduces wait times for patients.
Pharmacist:
Gigi Nazmy
Our Total Health Pharmacist provides counselling services to people who have questions about medications, herbal products or any over the counter products. Our Pharmacist helps to ensure people are using medications safely and effectively. Our Total Health Pharmacist is a certified compression stocking fitter and a certified Sisu recommender.
