At Prime Care, our Health Care Providers are offering the following:

To schedule your visit with our Health Care Providers at Prime Care FHT, please contact your physician’s office today.

After you have booked your appointment, please expect an initial telephone call from your Health Care Provider. Your health care needs will be discussed at that time in order to determine what type of visit (virtual or in-person) you require.

PLEASE NOTE – All counselling appointments with our Social Workers, Psychologist and Registered Dietitian will be conducted VIRTUALLY until further notice.

Diabetes

Are you living with Prediabetes or Type 2 Diabetes?

You are invited to join Prime Care FHT’s virtual 
Living with Diabetes 
upcoming online classes.

Whether you are newly diagnosed, seeking a refresher, or supporting a loved one, the Living with Diabetes classes will help empower you with the knowledge and skills to manage well. A Registered Diabetes Nurse Educator and Registered Dietitian will lead you through topics including what it means to have diabetes, medications, blood sugar readings, eating well and being active to better manage blood sugar levels.

This is a two-part series and signing up will automatically enroll you in both classes.

Class 1: Tues Dec 1st at 2:00pm-3:00pm
Class 2: Tues Dec 8th at 2:00pm-3:00pm

Classes will be held virtually through Zoom.
